This NCMD thematic report aims to analyse deaths of infants, children, and young people with life-limiting conditions and palliative and end of life care needs. It presents national findings on children with life-limiting conditions and analysis on the learning from the child death reviews on service provision. The report makes recommendations for improvements in commissioning services, access to medicines and professional training.

The authors of this report wish to acknowledge that the death of each child is a devastating loss that profoundly affects bereaved parents as well as siblings, grandparents, extended family members, friends and professionals.
